Workshop Leader Application
Please read thoroughly and answer each section completely.
Name (or Name of Group)
BEST Phone Number
Make sure that you list current contact information in order to reach you if your workshops are chosen
Example: Jane Leader 1234 Anystreet Delton, MI 49046 email@example.com (269) 623-5555
Please list the titles and descriptions of the workshops you’d like to offer. If your workshops are chosen for our folk school this year we will be using these titles and descriptions for the program so be sure your titles describes the workshops well for those looking at a glance, and your descriptions are clear without being too long. A title like “It’s a Colorful World,” for example, is not descriptive enough for a tie dye workshop. Try to be somewhat straightforward without being too dull.

Please check all that apply, and describe any needs in the comment section below.
I have specific needs regarding accessibility, tables and chairs, sound interference, electricity, etc. I will provide a description in the comment section below.
My workshop/s require expenses for which I will want reimbursement, as in the case of simple supplies such as paper, pencils, beads, thread or yarn. I will provide an estimate of these costs in the comment section.
I plan to vend at the festival as well as teach a workshops for the folk school. IMPORTANT (You will need to apply separately to become a vendor. See the vending tab at buttermilkjamboree.org. Please tell the folk school advisory committee what it is you would like to vend and whether or not you have submitted your vending application in the comment section below.)
